US CLUB SOCCER REQUIREMENT
For coaches for the teams playing on NorCal Premier League
1. Sideline Sports Doc/US Club Soccer course  -- make sure to put the code to avoid $5 charge
  1. Go to http://www.sidelinesportsdoc.com/usclubsoccer/
  2. Click the red box that says "Take The Course Today"
  3. Have a look at the Intro video and course description to get an idea of what you'll be learning, then click on "Signup!"
  4. Enter the info requested by US Club Soccer and when done click "Create Account"
  5. On the next page your coaches and staff will enter this code and will then bypass the payment portal, taking them to the course at zero cost to them:  Please email kay@sfge.org for the code

2. US Club soccer background request -- you need to complete Sideline Sports Doc online course before you can request background check -- you will need the confirmation number to proceed.
  1. Go to https://usclubsoccer.sportngin.com/register/form/502067227
  2. The club will reimburse you for your background check cost
  3. After you complete it, please forward the confirmation email kay@sfge.or or michelle@sfge.org 
CCSL CalNorth LEAGUE/RECREATIONAL LEAGUE REQUIREMENT
For the coaches for the teams playing on CCSL prep league or recreational league by SFYS
1.  League registration via Affinity  -- Please email office admin for the code.  Use the Registration link to register as a coach.
2.  Complete LiveScan
      Fill out the request for Livescan form below

3.  Email the LiveScan receipt to the office admin
4.  Complete Concussion course.  Email the concussion certificate to the office admin.
